Ontario court forms are strict about how an affidavit is sworn. A missing jurat, a wrong date or a signature that was not witnessed can get a document rejected at the counter. Our lawyer-commissioners swear court affidavits every day and know what the registrar expects.
Documents we commission
- Family Law Rules forms: financial statements (Form 13 and 13.1), affidavits in support of claims (Form 35.1), affidavits for divorce (Form 36) and general affidavits (Form 14A)
- Superior Court of Justice and Divisional Court affidavits under the Rules of Civil Procedure
- Small Claims Court affidavits and affidavits of service
- Ontario Court of Justice and provincial offences documents
- Affidavits of service, affidavits of documents and affidavits of execution
- Immigration and refugee proceeding affidavits
A note on legal advice
Commissioning an affidavit means confirming your identity and administering the oath; it does not include reviewing your case. If you want a lawyer to review the substance of your document or represent you, Law Wise Professional Corporation in the same office practises family, criminal, immigration and civil litigation.
Fees
Notarization from $30 per document. Ask us about a discount when you bring several documents in one visit. Drafting is available; the fee depends on the document and may involve additional charges, so please tell us what you need drafted and we will quote before any work begins.
