OSAP, School and Government Forms Notarized in Mississauga

Need an OSAP, school or government form notarized in Mississauga? Plenty of everyday forms carry the line “sworn before me” or ask for a notary’s seal. Bring the form; we check it, witness your signature and complete the commissioner’s section. Students and newcomers are our most frequent visitors, and we know these forms well.

Forms we see most often

  • OSAP: statutory declarations of income, marital status, independence and identity
  • University and college forms: name affidavits, residency declarations, lost-diploma declarations
  • Canada Revenue Agency and Service Canada declarations
  • WSIB and insurance declarations
  • Pension and life certificates (“proof of life”) for Canadian and foreign pension plans
  • Professional licensing and exam applications (PEBC, NCLEX, engineering, accounting and others)
  • Passport guarantor and identity declarations
  • Vehicle ownership transfers and ServiceOntario declarations

Frequently asked questions

Where can I get an OSAP affidavit or statutory declaration sworn in Mississauga?

Walk in to Notary Point at 33 City Centre Drive, Suite 606, beside Square One. Bring the completed, unsigned OSAP form and your photo ID; a lawyer-commissioner swears it while you wait, from $30.

Can you complete the commissioner section on a Service Canada or CRA form?

Yes. Bring the form unsigned. We witness your signature, administer the oath or affirmation where the form requires one, and complete and stamp the commissioner or notary section.

Do you notarize a pension life certificate or proof-of-life form?

Yes. Bring the form and your photo ID. Some foreign pension authorities require a notary public rather than a commissioner; our lawyers are both, so either requirement is covered.
